Axel Disasi is set to leave Chelsea this summer, with Leeds United and several Premier League sides paying close attention to his time at West Ham.
The 28-year-old joined the Hammers on loan in February and has played nearly every minute of their remaining Premier League games since.
Disasi has been unable to spur on a drive towards safety of late, however, with West Ham now in serious danger of the drop.
Daniel Farke’s side will travel to the London Stadium on the final day, while Tottenham will host Everton in their own attempts to stay up.
Leeds transfer chiefs will be keeping a close eye on developments, with Disasi’s future almost certainly dependent on the outcome.

Football Insider exclusively revealed last week that Disasi is keen to stay at West Ham permanently, but that a deal hinges on their Premier League status.
The centre-back has enjoyed an impressive impact at the London Stadium, helping form a defence that has been far more solid since his arrival.

The Hammers had lost just two of their opening 10 Premier League games with the Frenchman at the back and looked to be on the path to survival, but recent weeks have been damaging.
Disasi could do nothing to prevent defeats against Brentford, Arsenal and Newcastle United, leaving West Ham in an incredibly precarious position.
Considering the gulf in goal difference, their only hope of survival rests with Everton beating Tottenham on the final day, while the Irons beat Leeds.
But while, on paper, the Whites do not have anything to play for, their motivations may well lie with the potential ability to sign Disasi by sending the London club down.
West Ham squad to be picked apart
While Disasi’s future will have to be settled with Chelsea this summer, several other West Ham players are set to be the subject of significant interest.
Mateus Fernandes, Jarrod Bowen, El Hadji Malick Diouf and Konstantinos Mavropanos are already said to be attracting lines of suitors.
And former Leeds winger Crysencio Summerville will also expect to depart this summer if West Ham are indeed relegated.
The Whites accepted a bid of around £25million to let the forward leave in 2024, and they could be set to embark on a mission to see him returned to Elland Road.
